There is no single Anthem. Before you write a word, pull the provider manual for the state and product on the member's card, because the deadline, the form and even the name of the first step change with it. In California, file the Provider Dispute through Availity rather than the PO box whenever you can: Anthem's manual promises an acknowledgment letter within two business days for Availity disputes versus 15 working days otherwise, and that letter is your proof of receipt if the dispute stalls. Use the reprocessing request (Secure Messaging) only for obvious processing errors and never let it eat into the 365-day window. And remember that submitting the California PDR form "constitutes agreement not to bill the patient" while the dispute is open, so stop patient statements on that balance the day you file.

First, Find the Manual That Governs the Claim

Anthem publishes separate manuals by state and line of business, and its California and Georgia manual pages both say the contract wins: "In the event of a conflict between this manual and your contract, the terms of your contract will supersede the manual." To find yours, go to anthem.com, choose the state, open the provider section, and select Policies, Guidelines and Manuals. For California, that page currently lists the PPO Facility/Professional Manual (effective June 1, 2026), the HMO Manual (effective July 15, 2026), and the Medicare Advantage manual.

Three California routing rules catch billers off guard:

  • FEP PPO professional claims: the PPO manual says "For FEP PPO, Blue Shield of California handles the dispute resolutions for professional Providers. Blue Shield of California is not an Anthem affiliate."
  • HMO members in a medical group: financial responsibility is split by each medical group's Division of Financial Responsibility (DOFR), and the HMO manual says providers of non-capitated services may bill Anthem directly. If a medical group or IPA issued your remittance, start the dispute with that group.
  • Medi-Cal members: Anthem Blue Cross Medi-Cal has its own provider site, manual and timely filing policy (covered below).

Blue Cross of California: Filing the PDR Form

California's DMHC claims settlement and dispute resolution regulations (28 CCR 1300.71 and 1300.71.38) require Anthem to run a "fast, fair, and cost-effective dispute resolution mechanism" for participating and non-participating providers. When California billers search for the Anthem Blue Cross appeal form, this is the process they mean. Anthem's PPO and HMO manuals both set the window: disputes must be received "no later than three-hundred sixty-five (365) days from Anthem Blue Cross' action that led to the dispute," or, if Anthem never acted, 365 days after its time to contest or deny expired.

  1. Read the EOP and confirm the claim was finalized by Anthem Blue Cross, not a delegated medical group or Blue Shield of California for FEP.
  2. For a simple processing error, request reprocessing through Availity Secure Messaging or the customer service number on the member ID card.
  3. If reprocessing is denied, or the issue is a real disagreement, open the Anthem Provider Dispute Resolution Request (P_CA_00088, revised July 2020), the editable two-page Anthem provider appeal form PDF for California.
  4. Complete every starred field, including provider name and NPI, patient name, health plan ID number and service dates, add the original claim ID number, and pick the dispute type.
  5. Write a specific Description of Dispute and Expected Outcome, and attach support, but not a copy of a claim Anthem already processed.
  6. For substantially similar claims, list them on page 2 of the form, or batch them with numbered cover sheets and one cover letter.
  7. Submit through the Availity Portal, or mail to Anthem Blue Cross, P.O. Box 60007, Los Angeles, CA 90060-0007, the address printed in the manual and on the form.
  8. Track the acknowledgment (2 business days via Availity, 15 working days otherwise) and the written determination due within 45 working days.

What a California dispute must contain

The PPO manual requires the provider or facility name, tax identification number and contact information, plus a clear identification of the disputed claim, the date of service, and "a clear explanation of the basis for which the Provider or Facility believes the payment amount should be." Member-related disputes also need member names and ID numbers. Anthem may return an incomplete dispute, and the amended version is due within 30 working days of the returned receipt date.

Meet and Confer, then arbitration

If a participating professional provider still disagrees after the appeal decision, the next step is a Meet and Confer request to Anthem's Legal Department, then arbitration under the Agreement. The manual is blunt that "Meet and Confer and arbitration are not available to non-participating providers," and that the appeal and Meet and Confer "shall not toll the running of the applicable statute of limitations for filing an arbitration demand." Include the written appeal determination, your expected reimbursement, how you calculated it, and why you disagree.

The Two-Step Reconsideration and Appeal Outside California

Anthem's Georgia commercial manual (effective January 1, 2026) shows the two-step model. Use it as a template only after checking your own state's manual.

  • Step 1, Claim Payment Reconsideration: submit "via phone, Availity Essentials or in writing" within 365 calendar days of the EOP issue date, unless state law or your Agreement sets another period. You get one reconsideration per claim.
  • Step 2, Claim Payment Appeal: submit the Anthem Claim Payment Appeal through Availity Essentials or in writing within 90 days of the reconsideration determination. A reconsideration must be on file first.

Each dispute needs a position statement, your name, address, phone, email and NPI or TIN, the member's name and Anthem ID, the disputed claim numbers and dates of service, and supporting documents. Medical necessity and experimental denials are not claim payment disputes: they go through the clinical appeal track, which allows 180 calendar days from your receipt of the initial decision. A medical necessity denial such as CO-50 belongs in that clinical lane. A no-authorization denial such as CO-197 can go through the claim payment dispute when you did obtain an authorization and the authorized services match the claim.

Medicare Advantage Guidebook: 120 Days, Then 63

The Anthem Medicare Advantage provider guidebook (April 2026) gives participating providers a Claim Payment Reconsideration "within 120 calendar days from the date on the Explanation of Payment (EOP)," filed verbally, in writing or online through Availity. A Claim Payment Appeal follows "within 63 calendar days of the date on the reconsideration determination letter." Non-participating providers appealing a denied claim have 65 calendar days from the remittance notification and must include a signed Waiver of Liability holding the member harmless. The guidebook lists P.O. Box 60007, Los Angeles, as the written dispute address for California.

Medi-Cal Claims Appeals Through Availity

For Medi-Cal members, Anthem's California provider site says the appeal "must be received by Anthem Blue Cross (Anthem) within 365 days from the date on the notice of the letter advising of the action." File it in Availity from Claim Status: when a claim is denied or final, select Dispute the Claim to start.

So the Anthem Blue Cross timely filing limit in California is a contract question for PPO claims and a published number for HMO and Medi-Cal claims. For Medi-Cal, timely filing runs from the primary carrier's EOP when other insurance pays first. To overturn a CO-29 timely filing denial, Anthem's California manual accepts proof such as a claim copy with a computer-printed filing date, a billing system printout, or an EDI acceptance report showing the claim as accepted, and these appeals "must be submitted to Anthem in writing. Anthem doesn't accept appeals over the phone."

Corrected claims are not disputes

Fix billing errors with a replacement claim, not a PDR. Anthem California wants the entire claim resubmitted with claim frequency code 7 in CLM05-3 and the original claim number in REF F8, or frequency code 8 to void a claim billed in error, ideally through Availity. Rejected claims must be corrected and resubmitted within the timely filing period. Anthem says it will "re-categorize issues sent to us on a PDR form which are not true provider disputes," such as claim tracers or records sent after a no-documentation denial.
