RE: Appeal for Claim [Claim Number]

Patient: [Patient Name] · Member ID: [Member ID]

Date of Service: [Date of Service] · CPT Code: 97140

I am writing to formally appeal the denial of claim [Claim Number] for services rendered to [Patient Name]. The claim was denied under code CO-97, indicating that the services were considered to be part of a bundled service.

The manual therapy provided was performed on a separate body region from the therapeutic exercise, which justifies the need for distinct billing…

Is the appeal letter generator free?

Yes - it's completely free with no login or credit card. Generate as many payer-ready appeal letters as you need.

Does it store patient information (PHI)?

No. The generator never asks for or stores patient names, dates of birth, or member IDs. It produces a letter with bracketed placeholders you fill in locally.

Which denial codes and payers does it work with?

Any CARC/RARC denial code (for example CO-45, CO-97, or PR-204) and any payer such as Aetna, Cigna, UnitedHealthcare, or Blue Cross Blue Shield. Always verify the final letter against current payer policy before submitting.

Does it handle mental health and ABA denials?

Yes. When the CPT code or your description shows a mental health or substance use claim (psychotherapy 90785-90899, psychological testing, ABA 97151-97158, or behavioral health H-codes), the letter adds a federal parity request: it asks the plan to show that its prior authorization, review and medical necessity rules are no stricter than for medical care, and to provide the medical necessity criteria it used. Medicaid managed care plans get the Medicaid parity citations. Medicare claims are left out, because the parity law does not apply to Medicare.
